INFORMED CONSENT INFORMATION
Welcome to Kingdom Counseling Center. It is our desire to offer competent and compassionate therapeutic care to individuals, families, couples and children. Integral to this process is a commitment to work with personal issues toward the goal of healing and resolution. Therapy is a joint effort between the client and the therapist. It is an investment in one’s personal life and relationships to seek improvement, knowledge, experience and excellence in these areas.
Information disclosed by clients is considered confidential and will not be released to any third party without written authorization from the client, except where required or permitted by law. Exceptions to confidentiality include, but are not limited to, reporting child, elder and dependent adult abuse, when a client makes a serious threat of violence toward a reasonably identifiable victim, or when a client is dangerous to him/herself or the person or property of another.
It is our policy to collect fees at the time of service. Our agency fee is $95.00 per therapeutic hour which customarily runs from 50-55 minutes. Sessions lasting longer will be adjusted accordingly. Therapists accept cash, checks or Visa, which can be given directly to the therapist at the beginning of the session or to the receptionist at the front desk. From time to time, it may be necessary to engage in telephone contact other than scheduling appointments. The client will be responsible to pay for calls lasting longer than fifteen minutes at an agreed upon amount. Any call lasting the length of a therapy session will be considered a therapeutic hour and thus $95.00.
Clients can be given a “super-bill” upon request which provides all needed information for most insurance companies. Kingdom Counseling Center does not bill insurance thus keeping responsibility for payment and insurance reimbursements with the client.
Therapists ask that at least 24 hours notice be given for any cancellation of sessions. Clients will be responsible for payment of any sessions missed without notification. Cancellation notice can be given by calling the Counseling Center answering machine at (559) 226-1399 or by calling the receptionist at (559) 226-2750. These phone lines are available 24 hours a day.
Therapist’s offices are equipped with a confidential voice mail system which can be accessed at any time. Therapists will make every effort to return calls within 24 hours when possible, however, therapists are unable to provide 24-hour crisis service. In the event a client is feeling unsafe or in need of medical or psychiatric assistance, he/she should call 9-1-1 or go to the nearest emergency room.
Therapists reserve the right to terminate therapy at their discretion. Reasons for termination include, but are not limited to, untimely payment of fees, failure to comply with treatment recommendations, conflict of interests, failure to participate in therapy, and client needs which are outside of the therapist’s scope of competence or practice. This decision would be discussed with the client. The client also has the right to terminate therapy at his/her discretion. Sessions are intended to facilitate a positive experience of relationship and communication and thus, clients are asked to bring their decision to a session in which termination issues can be addressed. The therapist will also attempt to ensure a smooth transition to another therapist if desired.
